For practices using eClinicalWorks — access your health records, review lab results, request prescription refills, and communicate with your physician’s office securely.
Each physician manages their own patient panel and enrollment. Visit your chosen physician’s website or call our office to ask about current availability.
Submit refill requests via your patient portal or through your pharmacy. Allow 48 hours for processing.

How does concierge medicine work?
What is concierge medicine?+
Concierge medicine is a model where physicians limit their patient panel — allowing more time per patient, more personalized attention, and a higher level of care. A membership fee covers the enhanced access beyond what insurance provides.
Do you take insurance?+
We do not bill insurance for medical care provided through our practice. Our physicians operate membership-based practices, and your annual fee covers your care directly. We do recommend that patients maintain health insurance to help cover care outside of our practice, such as specialist visits, hospital care, imaging, labs, and medications.
How do I become a patient?+
Visit the individual physician’s website to learn about availability and how to schedule a welcome visit. You can also call our shared office at 312-470-8990 for general guidance.
Are the physicians affiliated with a hospital?+
Yes. All physicians are affiliated with Northwestern Memorial Hospital and hold faculty appointments at Northwestern’s Feinberg School of Medicine, providing excellent access to specialists and coordinated hospital care when needed.

What if I have an emergency?+
